Taking practice tests without analyzing misses is like weighing yourself without changing diet or training. The number moves, but you do not know which lever to pull next.
Error analysis is the habit of labeling every wrong (and every lucky guess) with a specific cause, then practicing the fix. Students who only track total score often restudy everything. Students who track categories focus on the 20% of habits causing 80% of losses.
This guide gives you a practical categorization system, a worked review of sample misses, a simple log template, and remediation rules for each error type.
Content gap: You did not know the rule, fact, or method. Example: you never learned that |x| > 3 splits into two rays. Fix = learn the concept, then drill that item type.
Process error: You knew the idea but executed the procedure wrong. Example: you remembered to flip an inequality sign when dividing by a negative, then flipped it when you should not have. Fix = slow, deliberate procedure practice with checkpoints.
Careless / execution slip: You had the skill and a workable process, but misread, mistyped, or rushed. Example: the question asked for x − 2 and you bubbled x. Fix = habits (underline the ask, estimate, substitute back), not more content videos.
If you are honest, many “I was careless” labels are actually process errors. Use the subtypes below before you let yourself off the hook.
Misread stem: You solved a different question than the one asked (value of x vs. 2x; “except” questions; units).
Arithmetic / algebra slip: Correct setup, wrong computation (sign error, distribution, calculator entry).
Transfer / bubble error: Right work on scratch, wrong choice selected (especially when choices are rearrangements).
Time-pressure guess: You could solve it with 40 more seconds but abandoned early without POE.
False confidence: You never checked; a 10-second substitution would have caught it.
Tagging subtypes matters because the fixes differ. Misreads need a “box the ask” ritual. Arithmetic slips need mandatory checks on multi-step math. Time-pressure needs pacing drills, not another grammar video.
Suppose after a Math module you missed five questions. Here is how categorization turns noise into a plan.
Miss 1 — Linear interpretation: You treated 40 in B = 40w + 250 as “weeks until balance is 40.” Category: content gap (slope vs intercept meaning). Fix: drill interpretation formats; write slope/intercept sentences before choices.
Miss 2 — Systems word problem: Setup swapped the two rates. Category: process error (translation procedure). Fix: force “Let x = …, Let y = …” and one sentence per condition before algebra.
Miss 3 — Quadratic: Factored correctly but answered a root when the question asked for the product of roots. Category: careless-misread. Fix: underline the final ask; pause one beat before selecting.
Miss 4 — Percentage: Used 0.2 instead of 1.2 for a 20% increase. Category: content gap (percent increase structure). Fix: short percent lesson + 10 targeted items.
Miss 5 — Easy arithmetic with correct method: 7 × 8 written as 54. Category: careless-arithmetic. Fix: estimate magnitude; re-check last computation line.
Pattern: 2 content, 1 process, 2 careless. Next week is not “study all of Math.” It is interpretation + percent content, one systems setup drill, and a check-habit goal on every solve.
Use a note or spreadsheet with these columns: Date | Section (RW/Math) | Topic | Question idea (5–8 words) | Bucket (content/process/careless) | Subtype if careless | One-sentence cause | Fix for next time | Re-solved cold? (Y/N).
Rules: fill the log the same day as the session. “I don’t know” is not a cause — re-read the explanation until you can finish the sentence “I missed this because ___.”
Weekly review (15 minutes): count buckets. If careless ≥ 50%, put habit work first. If content ≥ 50%, schedule topic sessions before more full tests. If process dominates, practice procedures with fewer questions and more narration of steps.
After three full tests, patterns stabilize. One test is anecdote; three tests are a plan.

Content gap: Identify the smallest concept (not “all of Algebra”). Do a short teach-back (explain the rule in one minute), then 10–15 focused questions. Stop when accuracy is high, not when you feel tired.
Process error: Write the procedure as numbered steps once. On the next five items, force yourself to follow the checklist aloud or on paper. Speed returns after accuracy.
Careless-misread: Physical ritual — underline or highlight the quantity requested before any work.
Careless-arithmetic: Substitute the answer back or estimate: “should this be near 10 or near 100?”
Careless-time: Flag and move at a set second threshold; return with POE. Practice modules with a visible timer so the threshold becomes automatic.
Do not remediate with random difficulty. Match the drill to the diagnosis.
On Math, content gaps are usually named skills (factoring, circle equations, percent increase). Process errors show up as correct method chosen but wrong order of operations, incomplete systems, or inequality sign mistakes. Careless errors are computational or “solved x but asked for 2x.”
On Reading & Writing, content gaps include grammar rules you never learned and vocabulary you cannot unlock from context. Process errors include reading the whole passage when the question is line-reference only, or eliminating answers without a stated flaw. Careless errors include missing “EXCEPT,” “most nearly means,” or evidence that contradicts a tempting choice.
Use the same three buckets in both sections, but expect RW logs to mention “extreme language,” “out of scope,” and “wrong line reference” as recurring process labels.
Reading the correct answer and nodding without re-solving. Recognition is not recall.
Calling everything careless to protect ego. That guarantees the same content gaps return on test day.
Logging errors but never scheduling fixes. A beautiful spreadsheet that does not change tomorrow’s session is decoration.
Only reviewing wrong answers and ignoring slow correct answers. Items you barely got right are future misses under stress.
Changing strategy every week based on one bad section. Use multi-test trends.
